Clinical Director for Diabetes

Tony has twenty years of experience in general practice in inner-city West London, and seven years’ experience in leading innovation and improvement across North West London. He was a Diabetes UK Clinical Champion from 2017-2019, and has spoken at numerous diabetes and digital conferences.

He is passionate about improving outcomes for people living with diabetes or at high risk of developing type 2 diabetes.

His focus is on two main areas:

  1. Supporting patients make improvements in health and wellbeing. He has led the development and launch of the North West London REWIND programme, as well as being a strong advocate of low carb and intermittent fasting approaches.
  2. Making use of digital technology to support patients more effectively. Preventing Diabetes is the result of this passion. It provides patients with a reliable web source of information, the opportunity to receive targeted email and text support for their condition and health records access.

Jane Wheeler

Director, Local Care, North West London ICB

The Local Care Programme supports North West London-wide delivery of out of hospital services. This covers areas as diverse as Long Term Conditions – such as Diabetes, Discharge, Long Covid, and other services delivered outside hospitals. Jane has worked in North West London for nearly 10 years leading work on Mental Health, working within RBKC borough and supporting the Grenfell Tower Fire health response.

Jane’s priority is to ensure across the programme that we are making concrete changes which improve the experience and outcomes for people across North West London – which can be sustained.

She is especially proud of our work supporting earlier discharge through the virtual ward and that North West London was the first to get accurate diagnosis services for lung conditions started up again after the pandemic. 

Phil Brough

Phil Brough

Head of Long Term Conditions, Local Care, North West London ICB 

Phil oversees delivery of a proactive model of care supporting people at risk, and/or living with diabetes, cardiovascular disease and respiratory illness. Phil is also vice chair of Citizen’s Advice in Hammersmith & Fulham which supports over 7,00 clients with advice on welfare, debt, housing, employment and consumer rights.

Phil is passionate about the lived experience voice of people to shape the design of health care services, reducing health inequities and is an anti-racist ally.

Some of Phil’s proudest achievements are working with DHSC and HMT to secure £148m of funding for hospices to deliver additional NHS capacity during the Covid-19 emergency response: providing 4,800 beds days, 50,000 community contacts, as well as supporting the rapid implementation of a single point of access and 24/7 specialist advice. 

Ian Reddington

Programme Manager – Digital Lead Diabetes

Ian joined at the start of the project and is particularly interested in digital healthcare, behaviour change, physical activity and weight loss. He has over 15 years of experience in the National Health Service, Public Health, and healthcare consultancy.  Ian’s main interest is to improve population health using digital channels. He leads the implementation and operations of digital features including the website, integration, CRM, and digital health marketing tools.

Nithan Chandrakumar

Digital Campaign Manager

Nithan is an expert in web development and digital campaigning technologies both in public and private sectors which make this position very much fulfilled. He also has advanced knowledge in 2nd and 3rd-line technical support as he has worked as a Senior IT technician at NHS for more than 5 years.

He enjoys being part of the team which allows him to support the diabetes population in Northwest London but also helps him to support his Dad who has type 2 diabetes.

John Shanko

Project Officer

John works in the Diabetes Transformation Programme as a project officer and is involved with the development and design of the programme’s digital platforms. He is interested in the impact of digital innovations within the health sector and digital health marketing.

Benson Dibiah

Benson Dibiah

Project Support Officer

Benson joined the Cancer Data Team at St Guys and St Thomas Hospital in 2021, in which he was part of a team working to monitor and review  care pathways  for cancer patients. 

In his current role, Benson works closely with local boroughs, Primary Care Networks and GP practices to implement the NHS Diabetes Prevention Programme along with other diabetes programmes in North West London.

Engagement Officer

Dakarai works as an Implementation and Engagement Officer with local boroughs, Primary Care Networks and GP practices to implement the NHS Diabetes Prevention Programme along with Diabetes Enhanced Service objectives in North West London. He is also a co-lead of the Engagement Strategy for the I&E team.

Dakarai comes from a background of almost a decade in Project Management across different sectors.
