Am I eligible?
You are eligible for your local NHS Healthier You service if you meet the following criteria:
18 years or older Not pregnant Have high glucose levels - defined as having a blood test reading in the last 12 months with a HbA1c between 42mmol - 47.9mmol/l or FGP 5.5-6.9 mmol/l or you have a history of Gestational Diabetes Mellitus (GDM) |
How to join
If you meet the eligibility criteria above, you can access your local Healthier You: NHS Diabetes Prevention Programme service in the following ways:
- You can sign up through the Healthier You website. To do this you will need your NHS number, date of blood test and blood test reading.
- A referral from your GP or nurse (just ask them about the Healthier You NHS Diabetes Prevention Programme)
A referral from a Health Check professional.
What can you expect?
Your local Healthier You: NHS Type 2 Diabetes Prevention Programme service is free to all those at risk of type 2 diabetes. Once you have signed up to the programme you will select which format to attend the programme:
- Group: Our face-to-face group programme is run at a range of local venues. Meet like-minded people and attend group sessions each fortnight to learn about healthy living, share your experiences and reduce your risk together.
- Digital: Prefer a digital programme? Get free access to the Second Nature app where you will login each week and speak to your health mentor over the phone.
- Remote: Delivered though regular zoom or telephone calls. This programme is specifically designed to support:
- Clients with visual / audio impairments
- Women with a history of GDM
- Non-English speakers. Our courses can be delivered in Urdu, Punjabi, Bengali and a range of other languages.
